Advair was approved by the FDA in 2000 for long term twice daily treatment of asthma and chronic obstructive pulmonary disease (COPD). In 1996 the 28-week SMART (salmeterol multi-center asthma research trial) was initiated to compare the effects of Serevent with a placebo. Serevent is a drug very similar to Advair, but contains salmeterol without the anti-inflammatory steroid. The study found that 13 people in the Serevent group died as a result of serious asthma attacks brought on by the medication.

Concern about similar risks over the effects of Advair prompted the FDA to order Advair label changes in to Advair and Serevent in 2003. The black box warning, the strongest FDA-ordered warning short of a recall, informs patients about the rare, but significant risk, of suffering a fatal asthma attack brought on by the medication.

In 2004, whistleblower Dr. David Graham of the FDA cited Serevent one of the top five most dangerous drugs available on the market today. In response, an FDA advisory panel of 14 lung experts convened in July 2005 to determine whether or not Advair and Serevent were safe enough to remain on the market. In a unanimous vote the panel recommended that these drugs remain on the market. The group of medical experts believes that the benefits of these drugs far outweigh the risks for most patients.
