Bextra TENS

Recent drug recalls and warnings placed on non-steroidal anti-inflammatory drugs (NSAIDs), like Bextra, have been sparked by the cause of Stevens-Johnson syndrome and toxic epidermal necrolysis (TENS). Stevens-Johnson syndrome and toxic epidermal necrolysis are two variations of a skin disease onset by allergic reactions to drugs.

The majority of all TENS cases are caused by a reaction to a drug, most often sulfa antibiotics, barbiturates, anticonvulsants, such as phenytoin and carbamazepine, and certain (NSAIDs). A recent drug recall of the COX-2 drug Bextra was sparked by the incidence of Stevens-Johnson syndrome and TENS. Very few cases are caused by a bacterial infection, and in some cases, a cause cannot be identified. The disorder occurs in all age groups but is more common in older people, probably because older people tend to use more drugs associated with onset of TENS. TENS is also more likely to occur in people with AIDS.


Both disorders are extremely serious and can be life threatening. Like Stevens-Johnson syndrome, TENS features a similar blistering of mucous membranes, but in addition, the entire top layer of the skin peels off in sheets from large areas of the body.

In TENS, blisters break out on the mucous membranes lining the mouth, throat, anus, genitals, and eyes. Eating can be difficult and drooling may occur because of damage to the lining of the mouth making closing the mouth painful. Swelling of the eye, eye pain, and the sealing shut of the eye from pus are also common. The corneas of the eye can become scarred as well with TENS.

TENS may also affect the urethra, the opening through which urine passes. This can create urination difficulty and can be extremely painful. Sometimes the mucous membranes of the digestive and respiratory tracts are involved, resulting in diarrhea and difficulty breathing.

The skin loss in TENS is similar to a severe burn and is equally life threatening. Huge amounts of fluids and salts can seep from the raw, damaged areas. Infection is a huge concern to a person who has this disorder, as they are very susceptible at the site of exposed tissues. TENS infections are the most common cause of death in people with this disorder.

TENS is a potentially life-threatening medical emergency that requires prompt diagnosis and treatment. If drug-induced, the offending drug must be stopped immediately to prevent further TENS advancement. Other medications that are unnecessary should be stopped as well during treatment. Patients should be hospitalized and given highly specialized nursing care in a burn unit to prevent infection.
