Experts have discovered a serious defect risk associated with the Actavis fentanyl patch, which can put patients and their caregivers at high risk of direct exposure to fentanyl gel.  Exposure to the fentanyl gel can lead to serious adverse events including severe respiratory distress and even fatality. 

In light of this serious Actavis fentanyl patch defect risk, the makers of this product issued a voluntary Duragesic® recall of 14 lots of the patch in February 2008. Less than a month later, the company expanded the recall to include all lots of Actavis fentanyl patches sold in the United States.

All Lots in the U.S.

All Actavis fentanyl patches with an expiration date of May 2009 to August 2009 are affected by the recall. The patches come in multiple doses, including: 25 mcg/hr, 50 mcg/hr, 75 mcg/hr, and 100 mcg/hr. 

Some of the recalled Actavis fentanyl patches bear two different names. The outer carton holding the patches displays the company’s current name, which is Actavis South Atlantic, LLC.  However, the individual pouches that hold the Actavis fentanyl patch may bear the company’s old name, which is Abrika Pharmaceuticals. It is important to look for both names—Actavis South Atlantic and Abrika Pharmaceuticals- when trying to determine if you possess the recalled patches. 

What should I do if I use the Actavis fentanyl patch? 

If you currently use the Actavis fentanyl patch and are in possession of patches affected by the current recall, do not use these patches.  It is important to dispose of these patches immediately by flushing them down the toilet.  Experts caution against handling the product directly, but if you are exposed to the fentanyl gel, you should immediately wash the affected area with large amounts of water only.  If you begin to feel any adverse reaction to the Actavis fentanyl patch, seek immediate medical attention.
